This form should be used to record your apologies for non-attendance at the ACM on 28 September 2024.
First of all, complete the personal details and select your KCASA Category(ies) from the first menu.
If you have made a selection that does not include 'Club Member', just submit the form now and your - or your club's -apologies will be noted in the meeting minutes as appropriate.
If you have selected 'Club Member', before submitting the form, please select your club from the second menu and complete your Swim England membership number in the box below. If you are not sure of your number, it can be found here. Please note that without this information your apology will not be recorded.
